To ensure you are receiving all benefits, you must keep your account current and provide all necessary information in a timely manner. We will work hand-in-hand with you, to maximize your insurance reimbursement for covered procedures. Should an appeal for denied benefits be required, you must bring your current account so that we may continue pursuing maximum reimbursement for you. Please provide us your insurance information at least 3 days PRIOR to your surgery, using our online form, or through secure email, mail or personal delivery, so that we can provide a more accurate estimate for your down payment, expedite reimbursement and minimize your out of pocket payment on the date of service. If your benefits are expired or can not be verified prior to your surgical appointment you will be expected to pay in full on the date of each visit. Your estimated down payment may change on the date of service or you may end up with a remainder balance if the information we obtained from your insurance carrier was incorrect or not current on the day of verification.
